Adjust + Align ("we," "us," or "our Practice") is committed to protecting the privacy of your health information. We are required by law — including the Health Insurance Portability and Accountability Act (HIPAA) and California's Confidentiality of Medical Information Act (CMIA) — to maintain the privacy of your Protected Health Information ("PHI"), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
HOW WE MAY USE AND DISCLOSE YOUR HEALTH INFORMATION
A. For Treatment
We may use your PHI to provide chiropractic adjustments, physical rehabilitation, therapeutic massage, and nutritional counseling. We may share your information with other healthcare providers involved in your treatment, such as your primary care physician or specialists, to coordinate your care.
B. For Payment
We may use and disclose your PHI to bill and collect payment for the services we provide to you. This includes submitting claims to your health insurance company or other payer.

We may also use or disclose your PHI without your authorization for the following purposes:

USES AND DISCLOSURES REQUIRING YOUR AUTHORIZATION

Under California law (CMIA), we will not share your medical information with employers, marketers, or for most research purposes without your explicit written consent. You may revoke any authorization you provide at any time, in writing, except to the extent we have already acted in reliance on it.
YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION
You have the following rights with respect to your PHI:
Right to Inspect and Copy: You have the right to inspect and obtain a copy of your medical records. We may charge a reasonable fee for copies. California law generally requires we provide access within 15 business days of your request.
Right to Amend: You may request that we amend information in your record if you believe it is incorrect or incomplete. We may deny your request under certain circumstances.
Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI in the past six years (HIPAA) or five years (California).
Right to Request Restrictions: You may ask us to restrict how we use or disclose your PHI. We are not required to agree, except that we must honor a request to restrict disclosure to a health plan if you have paid for the service out-of-pocket in full.
Right to Confidential Communications: You may request that we contact you only in a specific way (e.g., only by mail to a specific address). We will accommodate reasonable requests.
Right to a Paper Copy of This Notice: You may request a paper copy of this Notice at any time, even if you received it electronically.
Right to Be Notified of a Breach: You have the right to be notified in the event of a breach of your unsecured PHI.
CALIFORNIA-SPECIFIC PRIVACY RIGHTS
California's Confidentiality of Medical Information Act (CMIA) provides you with protections beyond HIPAA. Specifically, we will not sell, share, or otherwise use your medical information for marketing purposes without your explicit written authorization. We will not disclose your records to your employer without your consent except as required by law. In the event of a conflict between HIPAA and the CMIA, we will apply whichever standard affords you greater privacy protection.
COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR) at www.hhs.gov/ocr
or by calling 1-800-368-1019. You may also file a complaint with the California Medical Board. We will not retaliate against you in any way for filing a complaint.
CHANGES TO THIS NOTICE
We reserve the right to change the terms of this Notice at any time and to make the revised Notice effective for all PHI we maintain, including information created or received before the change. We will post the current Notice in our office and on our website. You may request a copy of the current Notice at any visit.

SMS Terms and Conditions
Users can expect to receive messages regarding chiropractic appointment details, reminders, and limited-time discounts and promos when they opt in.
You can cancel the SMS service at any time. Just text "STOP" to the short code. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time, and we will start sending SMS messages to you again.
